Allied Health Services - Community Rehabilitation Service (CoRe)

The ADHB Community Rehabilitation Team (CoRe) is a service that provides multidisciplinary rehabilitation for adults. The client group is mainly for persons discharging from hospital following an acute event with a significant change in level of function and ability to participate in their pre-existing roles.

Some examples include:

  • Recent Stroke
  • Recent Amputation
  • Rehabilitation post neurosurgery
  • Significant deconditioning following a prolonged hospital stay
  • New diagnosis of neurological condition

Rehabilitation is offered in the home, community setting and/or our clinics.

The service aims to optimise client independence, participation and function in clients’ daily lives and support self-management.  Programmes generally last between 2-12 weeks.

Within this service we also deliver an Early Supported Discharge (ESD) model that is able to deliver a similar intensity to inpatient rehabilitation but in a community setting.

Our aim

To work in partnership with the client within their home and community to:

  • Identify and work towards rehabilitation goals
  • Develop support strategies for the client and their caregivers
  • Develop links with community support networks and groups to improve social participation e.g. Stroke Foundation 

What to expect

Initially an assessment is completed to identify goals and aims of rehabilitation.

Following this, rehabilitation is planned according to client needs and is delivered face to face and/or by Telehealth (via Zoom or Telephone). Rehabilitation may include Occupational Therapy, Speech Language Therapy, Therapy Assistants, Physiotherapy and Clinical Psychology.

Between sessions there will be self-directed therapy tasks to get the best outcomes from rehabilitation.

Plans and goals are reviewed regularly during the rehabilitation period.

If long term needs and supports are identified, the appropriate referrals are made prior to discharge. A discharge letter is sent to the client and GP.
